This is the workplan:We will develop and apply breakthrough technologies: virally-delivered gene therapy targeted precisely, and minimally invasively, to the region of the heart where it is needed. First, we will optimize a loss of function mutation (G628S) in the KCNH2, the gene that encodes the IKr channel, and therefore can be used to prolong APD and prevent VT/VF. The construct must be targeted  to the exit points of the VT/ VF reentrant circuit in the infarct region by using a noninvasive endo- and epicardial activation imaging system that we recently developed and validated. The system is based on body surface electrocardiographic mapping (BSM; 64 electrodes) in combination with thoracic CT or MRI-imaging. Third, we will target the G628S-KCNH2 construct precisely to the identified exit sites in pigs with myocardial infarction, using high precision catheter injection techniques, and will monitor the effect of the KCNH2-G628S construct (i.e. APD prolongation) at the injection site by combined non-invasive activation and repolarization time mapping.
